Walk-ins are welcome however, however limited at the 2016 Annual Conference but please stop to check. Please remember to fast overnight for the most accurate results.) Confidential results will be mailed directly to you so that you can share your results with your PCP.

Any active HealthFlex participant who cannot access a Quest Diagnostics facility or on-site screening may be eligible for the $100 HealthCash incentive and up to 120 Wellness Points if his or her personal physician completes and faxes the 2016 Physician Results Report Form. - Deadline July 31st.
In addition, any participant and/or spouse that is not able to take a 2016 Wellness Screening during 2016 Annual Conference or through Quest Diagnostics by July 31st and wants to reduce their health premium rate has the opportunity to complete their wellness physical through their own private healthcare provider by returning a completed 2016 Physical Completion Form to the Indiana Conference office no later than November 4, 2016 .
